U.S. Coast Guard Releases the Final Action Memo (FAM) and Marine Board Final Report of Investigation Regarding the Stretch Duck 7 Sinking with the Loss of 17 Lives

The Coast Guard’s Office of Investigations and Casualty Analysis (CG-INV) recently posted the FAM and Final Report of Investigation to the Marine Casualty Reports (uscg.mil) website. On July 19, 2018, the amphibious small passenger vessel Stretch Duck 7 sank during a heavy weather incident on Table Rock Lake near Branson, MO, resulting in the tragic loss of 16 passengers and 1 crewmember.

Norway Safety Investigation Authority (NSIA) Report Marine 2023/06 into the Circumstances Surrounding the Casualty and Loss of Life Onboard the Cruise Ship Viking Polaris

The Coast Guard’s Office of Investigations and Casualty Analysis (CG-INV) recently posted NSIA Report Marine 2023/06 to Marine Casualty Reports (uscg.mil) regarding a marine casualty involving the cruise ship VIKING POLARIS southeast of Cape Horn, Chile, which resulted in multiple injuries to passengers and the loss of one life on Nov. 29, 2022.

Report of the Investigation into the Circumstances Surrounding the Sinking of the Commercial Fishing Vessel Scandies Rose

The Coast Guard’s Office of Investigations and Casualty Analysis (CG-INV) recently posted a Report of Investigation (ROI) to Marine Casualty Reports (uscg.mil) regarding the sinking and loss of the commercial fishing vessel Scandies Rose south of Sutwik Island, AK resulting in the loss of five lives Dec. 31, 2019. This investigation was a formal Marine Board of Investigation (MBI) convened by the Commandant of the Coast Guard.

The Coast Guard is scheduled to conduct a formal hearing starting Thursday, Jan. 26, in Virginia Beach to consider evidence related to the fire and total constructive loss of the passenger vessel Spirit of Norfolk.
