The Coast Guard’s Office of Investigations and Casualty Analysis (CG-INV) recently posted a Report of Investigation (ROI) to the Marine Casualty Reports (uscg.mil) website regarding the fire and subsequent land-based firefighter fatalities onboard the roll-on/roll-off cargo vessel GRANDE COSTA D’AVORIO (IMO# 9465382) while conducting cargo operations at Berth 18, Port Newark, New Jersey on July 5, 2023.
The fire continued for five days causing an estimated $23 million in damage to the vessel and resulted in the loss of life of two Newark, NJ Firefighters. This investigation was a District Formal Investigation convened by the Northeast Coast Guard District Commander. As a result of this investigation, the Coast Guard issued five safety recommendations, six administrative recommendations, five findings of concern, and one Safety Alert. A Commandant’s Final Action Memorandum (FAM) detailing the Coast Guard response and actions on each recommendation is included in the ROI.
